Rather than fire the employee you find out that the employee needs training and coaching. The goal of any incident investigation is to prevent more incidents, gain insight in context, and not antagonize or p**s the workforce off, thus making your job harder, and not actually finding answers. Your goal is to benefit your organization not to cause more confusion, animosity, and blame (especially blaming the employee).
